
Application Process
We’ve created a supportive, enriching environment where adults can continue their personal development, build practical skills, and engage meaningfully with their community long after they’ve aged out of school-based programs.
Pre-Admission Process
Through our comprehensive approach, we strive to honor each participant’s unique abilities while supporting their journey toward greater independence.
- Individuals 21 years of age and older are eligible to apply to Achieve Academy.
- Applicants must have an intellectual/developmental disability, with proof in the form of a doctor’s written diagnosis or, if the individual is a current HS graduate, the applicant’s most recent IEP.
- The initial contact may be made by the interested applicant, the family, or a support coordinator.
- Our admissions team, which consists of the executive director, BCBA, and program instructor, reviews the required paperwork before scheduling a tour.
Required Paperwork
- The potential client’s current Individual Support Plan
- The potential client’s most recent physical, medical history, and current medications
- Results of the potential client’s NJCAT assessment, including tier information
- Results of the potential client’s completed PCPT (Person-Centered Planning Tool)
- If the potential client graduated from a public school or approved school for students with disabilities within the prior three (3) years, we require a copy of the last IEP.
- Achieve Academy’s Potential Client form results (sent electronically to the applicant/family)
- Any other pertinent information that would be helpful in determining admission
Criteria for Acceptance
- The potential client must be and must remain DDD and Medicaid-eligible as criteria for acceptance.
- Clients must show proof of diagnosis.
- Phone conversations with the potential client’s support coordinator will take place before a tour is booked.
- If deemed appropriate by the admissions team, a tour will be scheduled at a time mutually convenient for the admissions team, the potential client, the potential client’s family, and the support coordinator.
- Following the tour, the admissions team will meet to review the potential client’s needs and Achieve Academy’s ability to meet those needs effectively.
